Minor Consent Form

For minors receiving services, this form will need to be completed with a Notary Public.

The parent/guardian must be present during the services provided to the minor and valid government issued identification must be presented. The minor must also have a government issued form of identification (social security card, birth certificate, state identification card, etc.)
